Enrollment Part 2: Choose Your Plan Options

In Part 2 of the EEZEE Choice enrollment process you will select your Protection & Benefits plan. You can choose between the easy Month-to-Month payment schedule or the money-saving Annual option. As an EEZEE Choice Participant your coverage will begin once your application and $75 enrollment fee is processed. There are no further payments for the first 3 months of coverage. Following this enrollment period your chosen monthly or annual* rate will be charged to your selected payment method. Make sure to fill all “required” fields and click the green “Confirm Your Plan Options” button to complete your enrollment process.

The Monthly Payment Schedule also begins at the end of the enrollment period. Both plans auto-renew at the end of their term for your convenience and uninterrupted coverage.
